We are in Surry County NC. I know we have had a harsh winter this year, but some of my Gulf Stream Nandinas look like they are thinning, losing leaves, and just plain look sick. We have several around our porch area that have been established for 4 yrs. One may look great, where the next one looks bad.

I've already cut back dead branches to just above the ground and am seeing some new growth starting to emerge. Same thing happened with my Pittosporum, Soft Caress Mahonia, some varieties of azalea, and a few loropetalums. Have pruned them all and am seeing new growth.

This winter was an exceptionally cold winter with the arctic cold that moved across the country. When temperature drop well below the cold hardiness of the nandinas their leaves will brown and drop off. Many plants we consider evergreen will lose some of their leaves in coler climates.
I know how ugly and alarming a defolicated Nandina can look like. Because of this cold weather and the damage I'm hearing of on many plants including the nandinas my guess regarding your defoliated nandinas was a result of winter damage and not any disease. As far as some looking worst than others, this can be caused by several factors. Some may be protected from cold winds or colder currents from other plants or structures such as your home.
Some may sit under an eve of the home while others are more out in the open. Some may be in a location that gets more water while some not so much rendering them more vulnerable to freezing damage of foliage and root systems. The nandina may take more time to recover from winter damage than many of your deciduous plants but most will probably recover. Waiting is all you can do for now. New growth should appear replacing the dropped leaves.
Occasionally some branch tips will die back from the cold. I would wait two or three more weeks and watch for any branches that haven't put on new growth. At this time you can cut back these branches to good living wood.
